Empowering people to monitor and improve their health using personal data and digital coaching

https://itea4.org/project/personal-health-empowerment.html

Project Description

Current care provision is reactive and process driven, treating patients according to predefined pathways with limited possibilities to take into account the individual needs or abilities. Health authorities and care providers are finally noticing the one resource that had remained unused, the person or patient him/ herself! Significant cost reductions can be achieved by preventive solutions to help the person adopt a healthy lifestyle.

The Personal Health Empowerment project aims to achieve this goal by empowering people to monitor and improve their health using personal data and digital coaching.

IWISH

Intelligent Workflow optimization and Intuitive System interaction in Healthcare

https://itea4.org/project/iwish.html

Project Description

Within a hospital, many departments and persons need to collaborate and communicate effectively when diagnosing and treating patients. Because of the complexity of patient’s symptoms and diseases, diversity of the organization within the hospital, of the equipment and of other factors, many workflow steps can be optimized. Typical problem areas are: (1) not all relevant patient data is available at the desks where people have to decide on diagnosis, treatment planning and treatment, (2) not all equipment is available, varying from surgery tools, infusion pumps, imaging equipment or the OR itself, or (3) not all required staff is available or in time for instance because previous procedures took longer. Or they have to wait because the next procedure has been delayed. This leads to errors and inefficiencies that can be addressed by making the right data available at the right place and time during an intervention, and by using the data for retrospective analysis and feedback.

TREAT

Transforming Healthcare Through Semantic Interoperability & Patient Self-Efficacy

https://itea4.org/project/treat.html

Project Description

TREAT aims to increase patient self-efficacy in managing non-communicable diseases including diabetes, heart attacks, cancers and chronic respiratory diseases, by using an asynchronous model and integrating data from wearables, journals, and medical records. In such a model, healthcare access moves from the clinic (system-centric) into the patient's daily life and health management shifts from the clinician to the patient.
